i. I solemnly affirm that I am not in the employment of any University or
College or of any Private or Public Sector.
ii. I solemnly affirm that, I have no Previous Criminal record and I have not
been subjected to any disciplinary action by the Proctor/Principal/Head of the
Department.
Instructions for Principals/Head
Principal/Head has to issue these certificates in his/her Signature and Seal.
i. Self attested High School Certificate/Marksheet as valid proof of the Date of
Birth.
ii. A certificate of 75 percent of attendance duly signed by the Principal/ Head
of the Institution/Head of the Department College/Department.
iii. A certificate from College/Department Controller of Examination declaring
that the candidate has no academic arrears in the contesting the election.
